User Access Request Form

Want to meet with your counselor or attend a group meeting online? 

Please complete the User Access Request form below for access to the LRM Portal.  Please choose your own username and password, and keep it for your records.  Once your account has been approved, you will receive a confirmation email to the address that your provide.  The confirmation email will also provide you with login instructions and the schedule of the online sessions and appointments.  Valid and Current Email Addresses and Phone Numbers are required to participate in this service.
