Recovery Support Services (RSS) Intake and Assessment

Please complete the attached intake packet it and return it to the access site with a copy of the consumer's state issued identification, social security card and proof of residency (if required). 


The Recovery Support Services (RSS) enrollment process takes approximately 90 minutes to complete from beginning to end.  Dismas House of Kansas City is offering enrollment appointments in two ways, face-to-face appointments and telehealth** appointments.   

The enrollment process generally includes the following: 

  • Screenings (Cage-aid, Mental Health and Depression)
  • Intake Meeting (GPRA Interview, Overview of RSS providers and services, Contacts/Consents to Contact and explanation of Recovery Management Checkups)*
  • Clinical Assessment (Treatment Level Recommendations) **
  • Provider Connection Agreement

 

*The intake meeting can occur over the phone or face-to-face. 

** Dismas House of KC is now offering telehealth appointments where the clinical assessment is provided online through a  video conference format.  Consumers must be present in a contracted RSS Provider or certified Recovery Residence to receive a telehealth appointment.  Consumers may not receive a telehealth visit from their home or personal mobile devices.  Contact the office for more information regarding a telehealth visit.
